Required Forms

You must submit these forms before August 1 to the Ridgewood High School School Nurse

1. Medical form (follow link to HS forms page) - Required

2. Compliance to RHS Hazing Policy  - Required

3. Compliance to RHS Substance abuse policy  - Required

4. Emergency Contact Information completed by a parent or guardian  - Required

5. FIT 

6. Fitness Center

7. NJ State Steroid Policy

When you submit the forms, you must place the Hazing Policy, Substance Abuse Policy and Emergency Card in an envelope addressed to Coach Mahler and attach it to the Medical Form.
